​​Welcome to the Provider Application Portal

Follow these stages in order to get approved and scheduled to see Veterans. Each tab in the header matches a step—finish all items in a stage before moving to the next.

1.) Initial Application: Complete Basic Info, NP/APRN Info, Licenses, Qualification Info. Upload a current CV, photo ID, NPI, and (if applicable) board certifications. Click Save/Submit.

2.) Pre-Interview Docs: In Files/Photos, upload clinic photos (exam room, waiting area, restroom with ADA rails/sink, posted evacuation map). Upload Malpractice COI, W-9, and NPDB Self-Query (official PDF). Sign any Adobe Agreements shown.

3.) Interview:  Use the email link to book your interview. Be ready to confirm scope (C&P exams—no treatment/prescriptions), clinic setup, and availability. Complete any follow-ups afterward.

4.) Credentialing & Contracting:  Finish vendor packets (Optum/QTC/VES as assigned) and attestations. Complete the virtual ADA/OSHA survey and address any fixes. Ensure your Site Status = Approved and all contracts show Signed/Complete.

5.) Training:  Create/sign in to Train.org, add your group code, complete the modules, download the certificate, then upload it under TRAININGS. In the TRAININGS tab, check off each item so all display Complete.

6.) Payroll:  After TRAININGS are complete, open PAYROLL to finish direct deposit and tax forms. Confirm payee details and address. Set your availability so Scheduling can place Veterans on your calendar.
